Default 55G For just one of my evaluationswn

I was scheduled for a sinus lift and implant on January 5th Nike Free Run 7.0 at Dr. Teodoro's office in Bonita Springs, FL office in . I known as the office 3 days prior to inform them I had not yet received my new flex benefit card and should I reschedule Ginger told me to create a check and the office would hold it until my flex benefit cards arrived. This was also discussed before my fianc the day of the procedure. I wrote a check for $3997 and it was cashed two days later. During surgery, Dr Teodoro explained to me the implant could not be placed. The assistant left during my surgery with my sinus subjected to tell my fianc how nice my ring engagement was. Following the surgery. I asked if they would be billing me for just $2150 for the sinus lift and Tiffany said "Yes, and we'll hold the check". The check was cashed 2 days later. When I went set for the followup I brought in my flex card and Tiffany said we "cashed the check, I'm busy now I decipher it later". I called again last month 1st to followup and she said it would be corrected took down my flex card number over the telephone to bill and said a cheque would be mailed. I waited on the week and my card was still not billed. I called Tiffany again using the problem and she said she was busy and would get to it. I received the attached email from the office after I called for the 15th time around the matter confirming the check could be in the mail. I have still not received the check
Work was supposed to bill my medical insurance for the sinus lift. I waited Thirty days and called and emailed, but nothing was completed. I was then told to file the claim and all that is needed is a procedure code. My insurance let me know a tax ID, diagnostic code and procedure code were needed. I've come across two another implant specialist's with xray and zeriox. I have been informed the sinus lift must be done again because there was not enough bone grafted. I have also informed work of this and nothing has been done.
My attorney has advised me to place a complaint with the BBB and Dental Board on this matter, as well as peruse legal action.

I was scheduled to have a sinus lift and implant. Paid $3997. The implant couldn't be placed. The office will not refund money for that implant or bill my insurance or provide me tax ID to submit myself for the sinus lift. I should have known better after i had the person checking out before me complain of a similar situation a few days earlier. I have since had the sinus lift checked out by two other implant specialists, both confirmed the sinus lift must be redone. Unfortunately, both Dr. Teodoro and his office staff do not know what they are doing or willing to resolve this situation.
