1. That the wife has been domiciled in the State of _________ at the time this action was commenced, and that domicile has been maintained for ninety days next preceding the making of the findings in this judgment, and this court has jurisdiction of the parties and the subject matter.

2. That the parties were lawfully joined in marriage on _________, 19__, and that the marriage was registered at _________, _________.

3. That two children were born to the parties during the course of their marriage; namely, _________, born _________, 19__ and _________, born _________, 19__. That no children were adopted by the parties and the wife is not now pregnant.

4. That the wife is the fit and proper person to have the care, custody and control of the minor children of the parties and the husband shall have liberal rights of visitation.

5. That the husband has been guilty of extreme and repeated mental cruelty without cause or provocation on the part of the wife.

6. That the parties have entered into a written agreement dated _________, 19__, which agreement the parties have found to be fair and equitable, and the terms of which have been approved by the court. The terms of the agreement have not been incorporated into or merged in this judgment for dissolution by agreement of the parties, but the parties shall be obligated to perform each and every term of the agreement in the same manner as if it had been incorporated in this judgment. Agreement has been filed as Exhibit A. [See §5.897.]

(a). That the bonds of matrimony existing between _________ and _________ are dissolved.

(b). That the agreement in writing between the parties dated _________, 19__ while not incorporated or merged in this judgment shall be binding upon each of the parties and they shall perform all of the acts undertaken by them in it.

(c). That the court is precluded from extending the period of periodic maintenance beyond the periods set forth in paragraph _________ of their agreement unless the parties agree to the contrary in writing.
